CGM Medical Consultation (only)

After making your payment, you will be directed to our scheduling page to book your appointment.

This medical consultation is intended to establish care with our medical team so that we can determine if prescribing a CGM (continuous glucose monitor) is appropriate.

This is its only purpose.

If you have other complex medical issues, we invite you to schedule a full, single-visit consultation instead.

For this service, you must reside in one of the states in which we are licensed. Those are CA, OR, AZ, TX, WY, ID, UT, OH, WI, IL, NY, VA, DE, FL.

This telemedicine visit is limited to only a medical history review and prescription of the CGM of your choice.

  • We can prescribe the FreeStyle Libre CGM or the Dexcom CGM.
  • Prescriptions are for a 6-month supply.
  • Your insurance may or may not cover all or some of the cost.
  • Payment will be at your pharmacy of choice.

If you opt to use Insulin IQ as a supplemental/specialty medical resource for your care, you fully understand that we recommend having a local provider for any acute illness or injury. If you do not have a local provider, then you acknowledge the need to be seen by an urgent care or ER for any acute illness or injury.

If you reside in one of the states in which we are currently licensed, Insulin IQ providers will be able to order labs and other diagnostic tests to help you improve your metabolic health. Our providers will also be able to work with you regarding your medications which includes addition, change or discontinuation. We will not be responsible for any acute care concerns such as infections or injuries. If you do not reside in one of the states in which we are licensed, our providers can give you recommendations. We WILL NOT be responsible for your medical care in these cases and will instead collaborate with your provider who oversees your medical care.

All medical services will be documented in a HIPAA compliant EHR (electronic health record) and you will have access to that medical record through a portal. All visits are cash pay only. We will not bill insurance on your behalf. If you would like documentation to submit to your insurance for reimbursement, we will be happy to provide that.
